The ADS Group is required to charge sales tax for all orders shipped to customers in Colorado, Indiana, New Jersey, Minnesota, Texas, and Wisconsin unless we have a valid Sales Tax Exemption certificate on file. For your convenience we've provided the necessary sales tax exemption forms below. You can fax the completed form to us at (763)449-5555. Or e-mail it to your ADS Group sales representative.
The Intellectual Property Rights Form is required to establish proof of copyright. The completed form must be provided for all new orders submitted for manufacture.
